(1) Roots. Any line with improper or worn seals will allow roots to grow through the seams.
(2) Improper installation of the drain line.

I received an estimate of $500 for toilet ring replacement, and stiff shower handle (parts included). Sounds like a lot as Google answer was about $150 for toilet only.
